Credit Card Authorization and Agreement with Miller Travel Company Authorization: I, the undersigned, hereby authorize Miller Travel Company to use the credit card information provided above for the purpose of booking, reserving, and prepaying for travel services as agreed upon, which are to be provided by third-party vendors. This authorization extends to all transactions involving travel arrangements made on my behalf by Miller Travel Company. Agreement to Terms and Conditions: I acknowledge and agree to adhere to the terms and conditions set forth by Miller Travel Company, as well as those stipulated by any third-party vendors involved in the supply of the agreed-upon travel services. I understand that the specific terms and conditions of service shall be communicated to me and require my acceptance prior to finalizing any travel arrangements. Payment Processing Acknowledgment: I acknowledge that the charge for the services might not appear on my credit card statement under the name of Miller Travel Company but rather under the name of the supplier and/or airline directly involved in the provision of the services. I further acknowledge that payment processing may take 3-5 business days to appear on my statement. Acceptance and Confirmation of Charges: By completing this form and indicating my acceptance—either through a digital signature, initialing, or checking a specified box—I confirm that the amount stated for the charge is correct. I authorize Miller Travel Company to charge my credit card for this specified amount and reaffirm my agreement to the terms and conditions related to this transaction. Travel Insurance Acknowledgment: I acknowledge that Miller Travel Company has recommended the purchase of a travel insurance policy to protect my travel investment against unforeseen events that may result in financial loss, including but not limited to cancellation penalties imposed by cruise lines, airlines, hotels, and tour operators. I understand the coverage travel insurance offers, including but not limited to Trip Cancellation, Trip Delay, Emergency Medical and Dental Expenses, among others. Should I choose not to purchase travel insurance, I hereby accept full responsibility for any financial loss incurred due to such a decision. Cancellation and Penalties: I acknowledge that cancellation penalties will apply should I decide to cancel my reservation. These penalties include fees imposed by the travel supplier and Miller Travel Company, as detailed in the Miller Travel Company Terms & Conditions. I understand that these fees are in addition to any supplier-imposed fees and are not covered by insurance. Once a payment for insurance has been processed, it is understood that the insurance fee is non-refundable. Final Confirmation Protocol: I understand that submission of my information via this form does not automatically confirm my reservation or process a charge to my credit card. I acknowledge it is my responsibility to review confirmation invoices sent by Miller Travel Company for accuracy and to ensure that all details conform to my expectations and requirements. I accept that Miller Travel Company is not responsible for cancellations or booking issues arising from credit card declines or any discrepancies that are not reported in a timely manner. Responsibility for Payment Compliance: I understand it is my responsibility to ensure that payment deadlines, as outlined in my booking details, are met and that any payment issues resulting from credit card limitations or declines do not absolve me of my financial obligations or the consequences thereof, as stipulated by Miller Travel Company’s policies.
